THE LEGACY CORPS PROJECT
The Legacy Corps for Veterans and Military Families (LCVMF) is designed to provide essential support services to veterans and military families, focusing on caregiving and community integration. This project utilizes trained AmeriCorps members to deliver in- home respite care, enabling caregivers to address personal and family needs while promoting the well-being of both caregivers and care recipients. The project will operate across selected sites in partnership with local veterans' organizations and community health services.
CURRENT PROJECT
30/30 TRANSITION PROGRAM
Our program supports low to moderate-income seniors aged 65+ as they transition from acute care to home. For 30 days post-discharge, we provide case management, home health, hospice, caregiving, and logistical support. We assist with medication, nutrition, housing, insurance guidance, and personal care needs, while promoting social connections and community engagement. We collaborate with hospital discharge planners for a smooth transition, track resource effectiveness, and monitor re-hospitalizations, all in alignment with California's Master Plan on Aging. If needed, we also help find alternative housing options.

INTERGENERATIONAL GARDEN
This innovative project will serve as a vibrant community hub where generations come together to grow not only plants but also relationships and understanding across ages. Our garden will be a place for local students, seniors, veterans, and the disabled to interact, learn, and thrive together.
